All new clients must complete and email or mail a signed copy of our intake form before any treatment is preformed. Additionally, to ensure appointments are kept on schedule, new clients are required to pay for your sessions at the time of booking.
We DO NOT bill Insurance companies of your behalf. Your insurance provider can assist in determining if your coverage will reimburse you for any out of pocket expense.

This is a "read only form". Download it, fill out the form, go to the "print" menu (as you normaly woud print something, choose "print" as PDF, this will create a PDF file of your information on the form. Email this to us.
You will receive an email from our office verifying we have received you paperwork. If your intake form is not turned in to our office before your appointment, comprehensive assessment can be longer.
